This article describes the combined use of the immediate dentoalveolar restoration (IDR) technique and an osseodensification implant site preparation method to improve immediate implant primary stability in periodontally compromised extraction sites. Positioning of soft and hard tissues was evaluated in two clinical cases in which the IDR technique and the osseodensification implant site preparation method were used to replace teeth at sites with severe alveolar bone loss. The results were analyzed by clinical assessment, photography, radiography, and computed tomography scans. Based on this preliminary study, the use of osseodensification can enhance the results achieved using the IDR technique due to improved primary implant stability, as measured by higher insertion torque. Osseodensification (OD) is a biomechanical bone osteotomy preparation technique that uses specially designed burs (Densah bur, Versah LLC, Jackson MI) that preserve and compact autograft alveolar bone in the osteotomy preparation site. OD has been reported to enhance bone density and bone-to-implant contact and also to increase implant insertion torque (IT). IT measured at time of implant placement is associated with primary mechanical stability. A failure to achieve primary stability results in implant micro-motion yielding lower implant success and survival rates. Low-density alveolar bone is associated with the lowest implant success/survival rates when implants are placed with conventional osteotomy preparation techniques. This study examined the effects of OD versus conventional osteotomy drilling technique on initial insertion torque and removal torque of dental implants placed in low-density human cadaveric bone.

Osseodensification is a novel biomechanical bone preparation to place a dental implant. It is a low plastic deformation of bone caused by rolling and sliding contact with a densifying bur that has flutes to densify the bone as it drills into it.

So far, the authors have placed 30 implants in 15 feline subjects up to a 3-year follow-up and no integration failures have been observed to date.

$Dental implants were a viable long-term treatment option in the 2 cats described in this article. The best approach seems to be immediate implant placement at the time of extraction to maintain the surrounding osseous structures and prevent collapse of the labial buccal plate that may result when a delayed placement approach is undertaken. With careful treatment planning and informed owner consent, implants placed into the canine area to replace a fractured or badly damaged tooth can restore the feline to normal function. Thus far, the authors have placed 30 implants in 15 feline patients (14 of which have been restored), monitoring these cases up to a three year period and no integration failures have been observed to date. Long-term results have shown implants and crown restorations can be maintained without adverse periodontal sequela or crown fracture. Continued follow-up of these cases is ongoing.$

The use of Densah burs in densifying mode can breach the sinus floor with autografting without causing any perforation. If further grafting is desired at the base of the sinus in appropriate cases, the densah burs can be operated counterclockwise and the graft can be delivered to the region. The use of this feature of densah drills with putty form graft material, as we have used, provides an effective and safe augmentation. These simplified minimally invasive antral membrane elevation technique can be an alternative for conventional technics. Considering the complications of the conventional methods, this new concept can give successful results in SFE with a more conservative and simple approach. Moreover increased implant stability is achieved due to osseous densification of the Residual Bone by Densah burs and it is seen as a significant advantage for the maxilla posterior region, where bone quality is generally low. Thus, the proposed technique could be recommended for sites with limited residual height

Osseodensification is a novel, biomechanical, non-excavation osteotomy preparation method. Unlike traditional drilling, osseodensification utilizes proprietary high-speed densifying burs to compact and autograft bone in its plastic deformation phase. The result is an expanded osteotomy with preserved and condensed bone tissue that maintains alveolar ridge integrity and allows for implant placement with enhanced stability.
